Case Manager - care planning (Philadelphia, PA)

We are growing our team at Lumen Corner Early Learning Group and adding a Case Manager whose main responsibility will be care planning. Low staff turnover is a goal we take seriously. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for meal programmes. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Philadelphia, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
