Harborlight Ivory Health Partners is looking for a Case Manager. The job centres on care planning for patient education, and it would suit someone who likes steady, practical work. You would join a small organisation of about 12 people based in Philadelphia. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries.
