Case Manager - care planning (Omaha, NE)

Northwind Drift Civic Services works with county and city offices around Omaha, and we need a Case Manager to lead our care planning. You would join a small organisation of about 31 people based in Omaha. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for case files. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Omaha, with some flexibility after the first few months. • We reply to every applicant.
