Case Manager - care planning (Oklahoma City, OK)

Summit Pike Learning Group operates schools and learning programmes around Oklahoma City. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 636 people based in Oklahoma City. What you will do • Help with applications and appointments for attendance records. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with attendance records.
