Case Manager - care planning (Oakland, CA)

Northwind Juniper Health Group is looking for a Case Manager. The job centres on care planning for clinic supplies, and it would suit someone who likes steady, practical work. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish.
