Case Manager - care planning (Oakland, CA)

Nova Nest Community Partners is a Oakland charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on care planning. Flexibility and kindness are expected. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for benefits enrolment. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Oakland. • We reply to every applicant.
