Case Manager - care planning (Oakland, CA)

Amberfield Cobalt Early Learning runs childcare centres in Oakland caring for infants, toddlers and preschoolers, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for enrolment. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, Monday to Friday. • On site in Oakland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
