Case Manager - care planning (Oakland, CA)

We are growing our team at Silverline Hollow Health and adding a Case Manager whose main responsibility will be care planning. We are busy and kind, and we look after each other. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Oakland. • We will tell you what to expect at each step of the hiring process.
