Case Manager - care planning (New Orleans, LA)

Fernwood Logistics runs classrooms and tutoring across the New Orleans area. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for learning materials. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in New Orleans, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
