Case Manager - care planning (New Orleans, LA)

Foxglove Granite Housing manages affordable housing communities across New Orleans, and we need a Case Manager to lead our care planning. You would join a small organisation of about 137 people based in New Orleans. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for resident services. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in New Orleans, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
