Case Manager - care planning (Nashville, TN)

Nova Beacon Community Partners is a Nashville charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 42 people based in Nashville. What you will do • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Nashville. • We reply to every applicant.
