Case Manager - care planning (Nashville, TN)

Foxglove Clear Care Services supports families and individuals in Nashville with personal care and coordination. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 992 people based in Nashville. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Nashville. • We reply to every applicant.
