Case Manager - care planning (Nashville, TN)

We are growing our team at Bright Hollow Care Services and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 69 people based in Nashville. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Nashville. • Applications are reviewed on a rolling basis.
