Case Manager - care planning (Nashville, TN)

Redstone Birch Care Services supports families and individuals in Nashville with personal care and coordination, and we need a Case Manager to lead our care planning. You would join a small organisation of about 153 people based in Nashville. What you will do • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Nashville, with some flexibility after the first few months. • We reply to every applicant.
