Case Manager - care planning (Nashville, TN)

Bluepeak Yarrow Civic Services works with county and city offices around Nashville. We are hiring a Case Manager to take on care planning. Benefits and stability attract long tenure. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for constituent requests. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with constituent requests. • Bilingual English and Spanish.
