Case Manager - care planning (Nashville, TN)

Redstone Ember Learning Group is an education provider in Nashville serving students and families, and we need a Case Manager to lead our care planning. You would join a small organisation of about 21 people based in Nashville. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for attendance records. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with attendance records. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Nashville. • Applications are reviewed on a rolling basis.
