Summit Meadow Housing Group is looking for a Case Manager. The job centres on care planning for lease compliance, and it would suit someone who likes steady, practical work. You would join a large organisation of about 207 people based in Nashville. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for lease compliance. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Nashville, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
