Case Manager - care planning (Nashville, TN)

Bright Fox Learning Group runs classrooms and tutoring across the Nashville area, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for learning materials. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Nashville. • Applications are reviewed on a rolling basis.
