Case Manager - care planning (Minneapolis, MN)

We are growing our team at Fernwood Collective and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for insurance authorisations. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Minneapolis. • Applications are reviewed on a rolling basis.
