Case Manager - care planning (Minneapolis, MN)

Summit Lakeside Health Group is a healthcare provider in Minneapolis with clinics and support teams, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Minneapolis, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
