Case Manager - care planning (Minneapolis, MN)

Bluepeak Maple Property Management is looking for a Case Manager. The job centres on care planning for rent collection, and it would suit someone who likes steady, practical work. You would join a small organisation of about 136 people based in Minneapolis. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for rent collection. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with rent collection. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Minneapolis, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
