Case Manager - care planning (Minneapolis, MN)

Rivermark Harbor Care Services supports families and individuals in Minneapolis with personal care and coordination. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish.
