Case Manager - care planning (Minneapolis, MN)

Westbrook Oak Health Group is looking for a Case Manager. The job centres on care planning for electronic records, and it would suit someone who likes steady, practical work. You would join a small organisation of about 92 people based in Minneapolis. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, Monday to Friday. • On site in Minneapolis. • Applications are reviewed on a rolling basis.
