Case Manager - care planning (Milwaukee, WI)

Highbridge Maple Property Management is looking for a Case Manager. The job centres on care planning for vendor invoices, and it would suit someone who likes steady, practical work. You would join a small organisation of about 48 people based in Milwaukee. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for vendor invoices. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with vendor invoices. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Milwaukee, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
