Case Manager - care planning (Milwaukee, WI)

Lumen Haven Learning Group runs classrooms and tutoring across the Milwaukee area. We are hiring a Case Manager to take on care planning. Families are partners. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in Milwaukee. • We will tell you what to expect at each step of the hiring process.
