Case Manager - care planning (Milwaukee, WI)

Nova Umber Civic Services supports public agencies in Milwaukee with budgeting, records and programmes. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for procurement. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with procurement. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Milwaukee, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
