Case Manager - care planning (Milwaukee, WI)

We are growing our team at Nova Pine Community Partners and adding a Case Manager whose main responsibility will be care planning. Mission matters here, and so does looking after staff. What you will do • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Milwaukee, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
