Case Manager - care planning (Milwaukee, WI)

Lumen Fox Property Management manages apartment communities around Milwaukee. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for vendor invoices. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Milwaukee, with some flexibility after the first few months. • We reply to every applicant.
