Case Manager - care planning (Milwaukee, WI)

Summit Amber Early Learning runs childcare centres in Milwaukee caring for infants, toddlers and preschoolers, and we need a Case Manager to lead our care planning. You would join a large organisation of about 350 people based in Milwaukee. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for enrolment. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Milwaukee, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
