Case Manager - care planning (Miami, FL)

Silverline Forge Care Services is a social-care provider serving older adults and people with disabilities around Miami, and we need a Case Manager to lead our care planning. The work is personal and so is the culture. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • On site in Miami. • We will tell you what to expect at each step of the hiring process.
