Case Manager - care planning (Miami, FL)

Lumen Lantern Property Management Partners manages apartment communities around Miami. We are hiring a Case Manager to take on care planning. Residents are neighbours, not numbers. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for resident events. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Miami. • We will tell you what to expect at each step of the hiring process.
