Case Manager - care planning (Madison, WI)

Silverline Pike Learning Group is an education provider in Madison serving students and families. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 69 people based in Madison. What you will do • Help with applications and appointments for school calendars. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Madison. • We reply to every applicant.
