Case Manager - care planning (Madison, WI)

Fernwood Sable Community Partners is looking for a Case Manager. The job centres on care planning for donor records, and it would suit someone who likes steady, practical work. Mission matters here, and so does looking after staff. What you will do • Help with applications and appointments for donor records. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Madison, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
