Case Manager - care planning (Madison, WI)

Bluepeak Stone Care Services Partners provides in-home and community support in Madison, and we need a Case Manager to lead our care planning. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, with a steady weekday schedule. • On site in Madison, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
