Case Manager - care planning (Madison, WI)

Ironwood Ridge Learning Group operates schools and learning programmes around Madison. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 183 people based in Madison. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication.
