Amberfield Meadow Community Partners is looking for a Case Manager. The job centres on care planning for volunteer shifts, and it would suit someone who likes steady, practical work. You would join a large organisation of about 973 people based in Madison. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for volunteer shifts. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish.
