Case Manager - care planning (Louisville, KY)

Foxglove Ivory Health is looking for a Case Manager. The job centres on care planning for insurance authorisations, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for insurance authorisations. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish.
