Case Manager - care planning (Louisville, KY)

Bluepeak Thistle Early Learning runs childcare centres in Louisville caring for infants, toddlers and preschoolers, and we need a Case Manager to lead our care planning. We celebrate small wins and steady routines. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for classroom supplies. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with classroom supplies. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Louisville. • We will tell you what to expect at each step of the hiring process.
