Case Manager - care planning (Louisville, KY)

Rivermark Meadow Community Partners is looking for a Case Manager. The job centres on care planning for client intake, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for client intake. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Louisville, with some flexibility after the first few months. • We reply to every applicant.
