Case Manager - care planning (Louisville, KY)

Lumen River Learning Group is an education provider in Louisville serving students and families. We are hiring a Case Manager to take on care planning. Collaboration between teachers and staff is strong. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for school calendars. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Louisville. • We will tell you what to expect at each step of the hiring process.
