Case Manager - care planning (Louisville, KY)

Highbridge Lantern Property Management is a Louisville property manager with leasing, maintenance and resident services, and we need a Case Manager to lead our care planning. You would join a small organisation of about 43 people based in Louisville. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for maintenance requests. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with maintenance requests. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Louisville, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
