Case Manager - care planning (Little Rock, AR)

Greenridge Willow Community Partners Group is looking for a Case Manager. The job centres on care planning for donor records, and it would suit someone who likes steady, practical work. You would join a large organisation of about 647 people based in Little Rock. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Full-time, with a steady weekday schedule. • On site in Little Rock. • Applications are reviewed on a rolling basis.
