Case Manager - care planning (Las Vegas, NV)

We are growing our team at Lumen Pike Learning Group and adding a Case Manager whose main responsibility will be care planning. Collaboration between teachers and staff is strong. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for learning materials. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Las Vegas. • We will tell you what to expect at each step of the hiring process.
