Case Manager - care planning (Las Vegas, NV)

Cedar West Care Services Group provides in-home and community support in Las Vegas, and we need a Case Manager to lead our care planning. You would join a small organisation of about 22 people based in Las Vegas. What you will do • Help with applications and appointments for licensing. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Las Vegas. • We reply to every applicant.
