Case Manager - care planning (Las Vegas, NV)

We are growing our team at Bluepeak Vista Community Partners and adding a Case Manager whose main responsibility will be care planning. We are small enough to know everyone's name. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Full-time, with a steady weekday schedule. • On site in Las Vegas. • We reply to every applicant.
