Case Manager - care planning (Las Vegas, NV)

Amberfield Keystone Care Services is looking for a Case Manager. The job centres on care planning for family communication, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Las Vegas. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Las Vegas, with some flexibility after the first few months. • We reply to every applicant.
