Case Manager - care planning (Las Vegas, NV)

We are growing our team at Highbridge Health and adding a Case Manager whose main responsibility will be care planning. We are flexible about schedules. What you will do • Help with applications and appointments for unit turnovers. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Las Vegas. • We reply to every applicant.
