Case Manager - care planning (Las Vegas, NV)

Bright Care Services is a social-care provider serving older adults and people with disabilities around Las Vegas, and we need a Case Manager to lead our care planning. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Las Vegas. • We reply to every applicant.
