Case Manager - care planning (Las Vegas, NV)

Foxglove Junction Community Partners Group runs community programmes across Las Vegas. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 12 people based in Las Vegas. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Full-time, Monday to Friday. • On site in Las Vegas. • We reply to every applicant.
