Case Manager - care planning (Las Vegas, NV)

Cedar Nest Community Partners Group is a Las Vegas charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our care planning. You would join a small organisation of about 19 people based in Las Vegas. What you will do • Help with applications and appointments for donor records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Las Vegas, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
